High school you are representing

Chaperone's name

Contact information (email and phone number of coach and chaperone)

As youth ultimate is new in Florida, players do not need to go to the same high school to be on the same team. However, you cannot team up for strength; only for numbers. ALSO, YOU MUST BE IN HIGH SCHOOL AND UNDER 19 TO PLAY. Next year, eligibility requirements will become stricter.

Price:

Since this tournament is sanctioned through USAU, there is a fee of $7.25 per player built into the cost of the event.

The cost of the event will be $200. However it will only cost $175 if you pay online on www.orlandoultimate.org by May 3rd.

Once you pay this, that is all you will have to pay. Included is water, lined fields, a full day of ultimate, snacks, and a chance to have your name put on our state trophy.
